Kindergarten Registration March 2 & 3

Registration will not take place at Eisenhower and Forest Park Schools.  For families living in the Forest Park neighborhood, registration will take place at the Forest Park Community Center located at 1017 Woodruff Road in Joliet.

Any child who will be five years old on or before September 1, 2017 must be registered.  An original state issued or certified copy of the child’s birth certificate and three proofs of residency are required (see below).  In addition, a physical examination and proof of immunization records must be submitted by the first day of school. 

The three proofs of residency must include one proof from Category A and two proofs from Category B.  Category A includes: most recent tax bill and proof of payment, mortgage papers (homeowners), signed and dated lease and proof of last month’s payment, letter from manager and proof of last month’s payment (trailer park residents), letter of residence from landlord in lieu of lease (District Form 7:60-AP2, E1 must be filled out), or letter of residence to be used when the person seeking to enroll a student is living with a District resident (District Form 7:60 AP2, E2 must be filled out).  Category B, showing proper address, includes: driver’s license or state identification, vehicle registration, voter registration, most recent cable television and/or credit card bill, current public aid card, current homeowners/renters insurance policy and premium payment receipt, most recent utility bill, receipt for moving van rental, or mail received at new residence.  For more information, call the Student Services Department at (815) 740-3196.
