FINAL STUDENT REGISTRATION UNDERWAY

 

Completed registration forms and proof of resident forms for any District 86 student may be dropped off at the following schools: Dirksen Junior High, 203 S. Midland Avenue; Farragut Elementary, 701 Glenwood Avenue; Forest Park I.E. School, 1220 California Avenue; Gompers Junior High, 1501 Copperfield Avenue; Hufford Junior High, 1125 N. Larkin Avenue; Sator Sanchez Elementary, 1101 Harrison Avenue; and Washington Junior High, 402 Richards Street.

Staff will be available at these schools on Mondays, Tuesdays, and Wednesdays through July 26, 2017.  The schools will be open on Mondays and Tuesdays between 8 a.m.–4 p.m. and on Wednesdays from 11 a.m.–7 p.m.

Parents/guardians of returning District students must provide proof of residency with one of the following documents: 1) most recent tax bill and proof of payment; 2) mortgage papers or signed/dated lease and proof of last month’s payment; or 3) letter of residence from landlord in lieu of lease.  In addition, two documents showing proper address must be presented.  These documents may include a driver’s license or state identification card, vehicle registration, voter registration, most recent cable television and/or credit card bill, current public aid card, current homeowners/renters insurance policy and payment receipt, or most recent utility bill.  New students must also provide identification for the parent/guardian, an original or certified copy of the child’s birth certificate, child’s previous school information, and legal custody documents if applicable.

Physical examination and proof of immunization records are also required for students entering pre-kindergarten, kindergarten, and sixth grade.  Eye exams are required for incoming kindergarteners or students attending District 86 for the first time.  Dental exams are required for students entering kindergarten, second, and sixth-grade and proof of TDAP immunization is required for all sixth-grade students.  For more information, please visit www.joliet86.org or contact (815) 740-3196
